ABSTRACT

【Objective:】 To explore the disease communication between cancer children and their parents from the perspective of their parents. 【Methods:】 Using qualitative description method and semi-structured interview, 16 parents from the pediatric oncology department of a tertiary A hospital were collected. Colaizzi 7-step method was used to analyze the interview data and summarize the theme. 【Results:】 Four themes were summarized, including perception of the psychological changes of children after illness, changes in the future planning of children, different attitude towards informing children’s diseases, different opinions on children’s participation in medical decision-making. 【Conclusions:】 The awareness of disease communication and medical decision-making in cancer families is insufficient. It is difficult for parents to communicate actively and effectively with their cancer children, and the children’s families are unable to make clear plans for the children’s future. Therefore, it is necessary to construct a theoretical system of disease communication and medical shared decision-making from the perspective of Chinese familism, and strengthen the promotion of scientific communication methods.

ABSTRACT

BackgroundPsychological capital has a significant impact on individual post-traumatic growth and well-being. However, there is limited research on the psychological capital and its related factors among parents of children with leukemia. ObjectiveTo explore the psychological capital status of parents of children with leukemia and analyze its influencing factors, in order to provide references for formulating intervention strategies to improve the psychological capital among these parents. MethodsFrom March 2021 to April 2022, 143 parents of children diagnosed with leukemia from the Department of Pediatrics at the First Affiliated Hospital of Xinjiang Medical University was selected. The general demographic information questionnaire, Positive Psychological Capital Questionnaire (PPQ), Chinese-Posttraumatic Growth Inventory (C-PTGI) and Index of Well-being Scale (IWB) were used to investigate the participants. Multiple linear regression analysis was used to analyze the influencing factors of psychological capital. ResultsThe PPQ total score for parents of children with leukemia was (113.48±15.80). The PPQ total score showed positive correlations with the scores of C-PTGI and IWB (r=0.727, 0.598, P<0.01). The course of children's illness (β=0.136, P=0.017), parental education level (β=0.139, P=0.012) and C-PTGI score (β=0.622, P<0.01) were influencing factors of psychological capital among these parents, collectively explaining 57.6% of the total variability. ConclusionThe level of positive psychological capital among parents of children with leukemia warrants improvement. Factors including course of the children's illness, parental education level and post-traumatic growth significantly impacted their psychological capital.[Funded by Innovation and Entrepreneurship Project for Graduate Students of Xinjiang Medical University (number, CXCY2022047)]

ABSTRACT

Objective:To explore the application effect of ADDIE (analyze, design, develop, implement, evaluate) teaching model in the nursing teaching of intensive care medicine.Methods:The data of 89 intern students who studied intensive care medicine in Cancer Hospital of Xinjiang Medical University between January 2016 and July 2020 were retrospectively analyzed. Intern students between January 2016 and December 2017 were included in control group ( n=43), and intern students between January 2018 and July 2020 were selected as observation group ( n=46). The interns in the control group were given regular teaching, and the interns in the observation group implemented ADDIE teaching model. The scores of core competence (comprehensive evaluation scale of core competence of intensive care nurses, CICCN), autonomous learning ability (assessment scale for autonomous learning ability of nursing college students), critical thinking ability (critical thinking disposition inventory-Chinese version, CTDI-CV) and evidence-based practice ability (Chinese version of evidence-based practice ability assessment scale for nursing students) were compared between the two groups of interns at the end of teaching. SPSS 19.0 was used for chi-square test and t test. Results:At the end of the teaching, the scores of CICCN scale, assessment scale for autonomous learning ability of nursing college students, CTDI-CV scale, and Chinese version of evidence-based practice ability assessment scale for nursing students were higher in observation group than those in control group (all P<0.05). Conclusion:The application of ADDIE teaching model in nursing teaching of intensive care medicine can effectively improve the core competence, autonomous learning ability, critical thinking ability and evidence-based practice ability of students.

ABSTRACT

@#Objective To investigate the clinical efficacy of low-frequency transcranial magnetic stimulation (rTMS) on post-stroke upper limb spasticity and its mechanism. Methods From September, 2015 to December, 2017, 23 patients with post-stroke upper limb paralysis were randomly divided into control group (n=13) and experimental group (n=10). Both groups received routine rehabilitation, and the experimental group received 1 Hz rTMS at primary motor area (M1) for eight weeks. They were assessed with modified Ashworth Scale (MAS), modified Barthel Index (MBI) and Fugl-Meyer Assessment-Upper Extremities (FMA-UE) before and after treatment, while the activation under fMRI in the task state was observed and the laterality index (LI) was calculated. Results The scores of MAS, FMA-UE and MBI improved after treatment in both groups (Z>2.121, t=6.248, P<0.05), and improved more in the experimental group than in the control group (Z>2.084, t=-2.095, P<0.05). The ipsilateral M1, ipsilateral sensory motor cortex and bilateral supplementary motor area were activated more in the control group than in the experimental group during the movement of affected hand. LI in the M1 increased after treatment in both groups (Z>2.366, P<0.05), and was more in the experimental group than in the control group (Z=-2.430, P<0.05). There was a positive correlation between the change of LI in the M1 and the improvement of the MAS and FMA-UE (r>0.612, P<0.05). Conclusion Low-frequency rTMS may improve the motor function and spasticity of upper limb after stroke by promoting reorganization of the cortex and inducing normalization of cortical function.

ABSTRACT

Objective To compare the differences of surgical site infection (SSI) between laparoscopic distal gas-trectomy (LDG) and open distal gastrectomy (ODG) for gastric cancer. Methods We set up strict inclusion and ex-clusion criteria. All the randomized controlled trials (RCT) on LDG and ODG for gastric cancer were collected. Meta-analysis was performed according to the recommendation by the Cochrane handbook. Results Six RCTs in-cluding 767 patients were analyzed, who were divided into LDG group (n =394) and ODG group ( n=373). Postop-erative wound infection and SSI were significantly lower in LDG than in ODG [RR=0.32, 95 %Cl (0.11, 0.91).P =0.03; RR= 0.28, 95 %Cl (0.12, 0.70),P =0.006]. There was no significant difference in intra-abdominal abscess between the two groups [RR=0.35, 95 % Cl (0.09, 1.31), P=0.12]. Conclusions LDG was associated with a lower incidence of SSI, especially wound infection, as compared with ODG in the meta-analysis.

ABSTRACT

Objective To explore the correlation between the cancer-related fatigue and uncertainty in illness among breast cancer inpatients. Methods Totally 90 patients with breast cancer were involved in the study by cancer fatigue scale (CRS) and Mishel's uncertainty in illness scale-adult (MUIS-A). The correlations were analyzed. Results Approximately 100.00%patients experienced fatigue during hospitalization. The total score of uncertainty in illness was 107.51 ± 21.26. There was a positive correlation between each fatigue dimension and uncertainty in illness (P<0.001). Conclusions Cancer-related fatigue is widespread among breast cancer inpatients and sense of uncertainty in illness of patients is at a moderate level. Nursing staff can help patients to reduce disease uncertainty and improve the life quality of patients by strengthening health education , providing comprehensive information support, fully mobilizing patients' social support system.

ABSTRACT

Objective To evaluate the quality of Radix et Caulis Ilicis Asprellae from Pingyuan planting base and Chinese herbal medicine market. Methods The water- and alcohol-soluble extracts from 19 batches of Radix et Caulis Ilicis Asprellae medicinal materials were detected according to Appendix ⅨH, ⅩA of the Chinese Pharmacopoeia ( 2010 edition). And the quality of the medicinal materials was evaluated by microscopic identification technology according to the method for Radix et Caulis Ilicis Asprellae recorded in Guangdong Provincial Chinese Medicine Standard, and then thin layer chromatography ( TLC) was optimized to establish the high performance liquid chromatography (HPLC) fingerprint. The HPLC was performed on Waters XBridgeTM C18 column (250 mm × 4.6 mm, 5μm) with acetonitrile(A)-0.2% (v/v) phosphorus acid (B) as the mobile phase by gradient elution, flow rate was 1.0 mL/min, and detection wavelength was 220 nm. Results The results of sample characters, TLC and microscopic identification showed that the samples of Radix et Caulis Ilicis Asprellae in Chinese herbal medicine markets were certified products, but stems and roots were blended. Seven common peaks were showed by HPLC and confirmed by similarity analytical software. The similarity of 15 batches of planting base samples was all above 0.9. Of 19 batches of the commercial samples, the similarity of 11 batches was above 0.9. The alcohol-soluble extract contents were in the range of 64.55 mg/g to 186.18 mg/g. Conclusion The medicinal materials of Radix et Caulis Ilicis Asprellae from Chinese herbal medicine market are certified products, but the qualities vary greatly for the blending of stems and roots and inadequate growth years. The quality of materials from planting base is better. The established method is helpful for the quality evaluation and control of Radix et Caulis Ilicis Asprellae.

ABSTRACT

Objective To investigate the distribution and drug resistance of common pathogens in Xinjiang ,aare so as to provide references for reasonable use of antibiotics .Methods The strains of common pathogens isolated from patients in the First Teaching Hospital of Xingjiang Medical University from 2012 to 2013 were collected ,and the drug susceptibility testing were performed by K‐B methods recommended by CLSI .Results Totally 18 374 strains were isolated ,among them 13 323 strains were gram negative and 5 051 strains were gram positive .Escherichia coli ,Klebsiella pneumoniae ,Staphylococcus aureus ,Acinetobacter baumannii and Pseudomonas aeruginosa occupied the top 5 .Most of strains were isolated from sputum (accounted for 36 .1% ) .Escherichia coli and Klebsiella pneumoniae showed high resistance rate to cefazolin sodium ,cefotaxime and quinolones .The detection rate of ESBLs pro‐ducing Escherichia coli and Klebsiella pneumoniae were 48 .4% and 41 .7% ,respectively .The resistance rate of Pseudomonas aerug‐inosa to commonly used antibiotics was 10 .0% ~20 .0% .Methicillin resistant staphylococcus aureus(MRSA) accounted for 44 .7%of all Staphylococcus aureus ,and no strains of Staphylococcus resistant to vancomycin ,teicoplanin and Linezolid were found .Conclu‐sion Gram negative bacteria are the most common strains isolated from clinical in this area ,and strains are mainly isolated from samples of respiratory tract and genitourinary tract ,and the situation of drug resistance is severe ,which indicate the clinicians should strengthen the monitoring of drug‐resistant bacteria and promote rational use of antimicrobial agents .
